Who We are

Community Health and Behavioral Services offers comprehensive assistance to people of all ages in need of behavioral health support. Our staff are trained to help children, adolescents and adults with a variety of health issues.

Our Team: Our team includes nurse practitioners, RNs, LPNs, psychiatrists, licensed social workers and care managers. Designed to improve functional abilities of people diagnosed with a variety of physical and mental health conditions, treatment is provided by New York State licensed physicians and clinicians.

What Does it Cost? We accept Medicaid, Managed Medicaid, (Fidelis, Blue Options, United Health Care), Child & Family Plus Plans, Medicare and most private insurance companies.  We also offer a sliding fee scale based on eligibility.  Our staff will assist you in finding the best method of payment to suit your needs.  Our goal is to ensure that ability to pay does not hinder your access to quality care. No one will be denied access to care based on inability to pay.

Care Management

Upstate Cerebral Palsy Care Management Program Provides personalized clinical and social support and service coordination aimed at improving the health and well-being of the community.

Our mission is to provide access and care coordination to health and community support services that will enable individuals to live healthier, more productive lives.

We know that navigating through different health care services can often be difficult and confusing.

Through our caring partnership, we can provide you with direct access to a wide network of health care and community support services and can coordinate all of this for you!

These services are at no cost to you and are a part of your Medicaid coverage.

What Type of Care Do We Offer?

You will be provided with a dedicated Care Manager who will help you with your health care and other community and support needs

Services Your Care Manager May Coordinate Include:

  • Medical Appointments
  • Mental Health Appointments
  • Links to substance abuse treatment
  • Help with your housing
  • Self-help and support
  • Special services
  • Home Care and nursing
  • Help with your Public assistance application
  • Food, Shelter
  • Any other identified area of need

How Do I Know If I Am Eligible?

Children and Adults are eligible for our services. If you are receiving Medicaid, Medicaid Managed Care (Fidelis, United Health Care, or Excellus), or are Medicaid eligible, reside in Herkimer county and surrounding areas and have been diagnosed with a chronic medical condition or have mental health concerns you may qualify for services. If you meet the eligibility criteria and would like to enroll, or have questions concerning eligibility, please contact (315)866-2863.

Other Supports

The Image Center

The Image Center provides a place for individuals looking for a safe place to socialize with others. Image center offers several activities including arts and crafts, snacks, and watching movies just to name a few. The Image Center also assists members with the practice of attaining certain skill-set goals like cooking, communicating, organizing, and cleaning. The most important thing the center brings is a smile to people’s faces when they walk through the door.

Mentor Program

Children are referred to Upstate Caring Partners by a mental health provider for mentoring services. These services include 1:1 outings within the community on a weekly basis to help with social skills and added support for those individuals that are struggling with their own mental health. Examples of these skills would be role modeling correct behaviors, assisting with homework, and socialization.

Family Peer Advocate

The family peer advocate program provides direct peer support services to families with children who have medical, mental, or behavioral challenges. Support specialist assists families who need to navigate the special education and mental health service systems. Services include education on caregiver and child rights and responsibilities, and attending planning meetings with families. Working collaboratively with parents or guardians, youth, schools, and other service providers. Family Peer Support assists to empower families and helps them participate fully in the development of their child’s care, so they can advocate on their own.

Peer Support Specialists

Peer Support Workers are individuals with lived experience and have been successful in the recovery process who help others experiencing similar situations. Through shared understanding, respect, and mutual empowerment, peer support workers help people become and stay engaged in the recovery process and reduce the likelihood of return to use or engagement in unhelpful practices. Peer support services can effectively extend the reach of treatment beyond the clinical setting into the everyday environment of those seeking a successful, sustained recovery process (SAMHSA, 2022)

Service members, veterans, and their families (SMVF) are often overwhelmed by the complexities of navigating the service systems designed to support them. While many resources are in place to support the health and well-being of SMVF, finding and accessing those resources can be challenging. The training and support for Peer Support Workers with military-affiliated experience can help bridge the gap between the SMVF member who needs resources and the organizations that are designed to meet those needs.
