Service Coordination (MSC)

Upstate Cerebral Palsy Medicaid Service Coordinators (MSC) work with eligible individuals 3 years of age and older to identify unmet needs, desires and future goals to assist in creating a plan of action for you or your family member. In doing so, strengths and ambitions are carefully considered along with the input of family members and guardians. Service coordinators reach out to area affiliates in securing the supports and services necessary to meet those needs. Eligibility for MSC is determined by The Office for People with Developmental Disabilities (OPWDD) and the New York State Developmental Services Office (DSO). People and their families can benefit from a variety of services to which the MSC provides linkage.

Some of the Upstate Cerebral Palsy programs and services available to you or your family member include: recreation and leisure programs, vocational opportunities in the community, habilitation skills training, home environment modifications, transition services from home living to independent community living, self-direction, assistive technology and equipment, individualized habilitation services, advocacy, benefits assistance and legalized guardianship services. MSCs can access and connect you or a family member to these programs to ultimately fulfill your needs.
