School-Based Health Center

The School-Based Health Center is a unique venture between Donovan Middle School and Community Health and Behavioral Services, a division of Upstate Cerebral Palsy.  With no out-of-pocket expense to students or their families, the School-Based Health Center provides comprehensive primary health care services exclusively to students of Donovan Middle School. 

Staffing includes: a full-time nurse practitioner, a part-time supervising physician, a full-time licensed practical nurse, and a medical assistant.

Services include: 

  • Sick child visits
  • Physical examinations
  • Nebulizer treatments
  • Immunization
  • Mental health screening and referrals
  • Oral screening for dental referrals
  • Diagnostic treatment of acute and chronic illness
  • Health education
  • Referral services
  • Vision, hearing and scoliosis screening
  • First aid
  • Counseling by referral
  • Social work services by referral
